Why do Cataracts Develop

Cataracts form when the proteins in the eye’s natural lens clump together, causing clouding and scattering of light, reducing vision. It occurs commonly with advancing age, but can also be present in the newborn (congenital cataract).

Modern Cataract Surgery

The only effective treatment for cataract is surgical removal of the cloudy lens, replaced with a clear artificial Intraocular Lens (IOL).

When to Consider Surgery

When cataract begins to affect daily life — reading, driving, recognizing faces etc — it is time to consider surgery. Waiting for cataracts to become ‘mature’ (as in olden days) is no longer recommended, as they can present challenges (and complications) during surgery with modern technology and one may have to resort to older style large-incision surgery with sutures. This can affect the outcome of the surgery.

Modern day surgery is with very small incision, and usually without any sutures – making it possible to use premium IOLs. This is responsible for quick recovery.

Modern Techniques Used by Dr. Pathak-Ray:

Phacoemulsification: Ultrasound-based lens removal through a tiny 2.2 mm incision.

Micro-incision Cataract Surgery (MICS): Minimally invasive, ensuring faster healing.

Premium IOLs: Tri-focal that correct distance, intermediate and near, or advanced monofocal that corrects distance and intermediate vision (laptop, computer screen, car dashboard are examples of intermediate vision) are offered. Standard IOL is also offered. So are toric IOLs which can correct astigmatism, in one procedure.

Sclera-fixated IOL: Dr. Pathak-Ray is one of a handful of anterior segment surgeons who is capable of doing these types of IOL in a minimally invasive manner (Yamane technique). This is done when there is no support for safely placing an IOL in the ‘bag’ of the natural lens. The white of the eye (called sclera) is used as a scaffold for placement of these multi-piece IOLs.

Recovery:

Most patients regain clear vision within 24–48 hours and resume normal activities within a few days.
Follow-up visits ensure proper healing and long-term clarity.

When Cataract and Glaucoma Coexist

In many patients, cataract and glaucoma co-exist as both are relatively commoner in advancing age..
Dr. Pathak-Ray specializes in combined cataract–glaucoma surgeries, which safely remove the cataract while controlling eye pressure in a single procedure. She will advise on which combined procedure – Phaco-MIGS (istent Inject/ KDB/ECP), Phaco-Trab or Phaco-Tube – is most appropriate.

This approach reduces the need for multiple surgeries and helps maintain both clarity and optic nerve health.

Furthermore, ‘mature’ and ‘hypermature’ cataracts can give rise to glaucoma. These too have to be managed, either with cataract surgery alone or combined cataract and glaucoma surgery.

The surgical plan is tailored individually, considering the type and severity of glaucoma.
